What They Are and What They’re Used For

Vestibular Evoked Myogenic Potentials, better known as VEMP, are a specialized, quick, and non-invasive diagnostic test used to evaluate the function of the vestibular system, which plays a crucial role in maintaining balance.

The test is based on the muscle response that occurs when certain areas of the inner ear are stimulated with sounds or vibrations. There are two main types of VEMP:

1. cVEMP (Cervical VEMP):
Evaluates the saccule and the inferior branch of the vestibular nerve. An electrode is placed on the sternocleidomastoid (SCM) muscle in the neck to record the activity.

Evaluates the utricle and the superior branch of the vestibular nerve. In this case, electrodes are placed below the eyes to record the response in the inferior oblique muscle.

How Is the Vestibular Evoked Myogenic Potential Test Performed?

The VEMP test is simple and comfortable for the patient. Depending on the type of VEMP, electrodes are placed either on the neck or below the eyes. These electrodes record the muscle responses to a sound or vibration stimulus. The process generally follows these steps:

  • Preparation:
    The patient lies down or sits while electrodes are placed in specific areas according to the type of VEMP being performed.
    For cVEMP, the patient must tense the sternocleidomastoid muscle in the neck.
    For oVEMP, the test is done while the patient looks upward.
    It’s important for the patient to maintain the correct posture and muscle contraction during the test to ensure accurate results.
  • Stimulation:
    Sounds or vibrations are applied to the ear using headphones or vibrators. The resulting muscle response is recorded and analyzed.
  • Recording:
    The electrodes collect the electrical signals generated in the muscles, which are then processed to produce a graph of vestibular activity.

 

What to Expect from the Results

VEMP results are interpreted based on the presence or absence of electrical waves, symmetry between both sides, and response amplitude.
A normal result indicates proper vestibular function, while any abnormalities may point to an issue within the vestibular system.

 

What Conditions Can Be Diagnosed with VEMP?

VEMP testing is useful for diagnosing several balance- and ear-related disorders, including:

  • Vestibular neuritis: May produce a reduced or absent VEMP response on the affected side.
  • Superior semicircular canal dehiscence syndrome (SSCDS): Causes an exaggerated VEMP response, helping to identify this condition.
  • Ménière’s disease: May show a reduced VEMP amplitude due to alterations in the inner ear structures.
  • Otosclerosis and vestibular schwannoma: These conditions may result in an absent response during testing.
